The term ‘formbuilding’ refers to the process of making web pages that have spaces for people to type information. This information is processed by the server where the webpage is located. This formbuilding program enables you to prepare your own multiple-choice, true/false or missing word online test. The test is uploaded to the Internet, and when students complete the test, they get immediate results, and you get a message showing their score.

Process...

  1. follow the instructions to prepare the worksheets
  2. become familiar with the use of the formbuilding program - Demonstration Version
  3. make the test using the formbuilding program (opens in a new window, do not close it until saved)
  4. follow the instructions to save the test
  5. check the online test by answering the questions from your original answer sheet
  6. if you don't get 100%, follow the instructions - Troubleshooting
  7. send a copy of the worksheet (a 'html' document at this stage) to me - merv@plasform.com.au
  8. nominate the email address you want the student responses sent to
  9. nominate whether you want the worksheet available for other teachers to use

The Worksheet Building program relies on form processing functions at Internet Service Provider level, so it is important to remember that your work cannot be saved until step four.

The designer of the program is a dedicated proponent of 'open source', so you can use it for free (see Conditions). In the spirit of open source, Plasform is happy to host your material, and direct all the student responses to your email address.
